Financial Policy
You are our priority. Our intent is always to treat you with our full attention and utmost care. To
that end, we have policies that allow us to give our all while you are in our care. Every effort will
be made to accommodate patients who arrive more than 15 minutes late, however, to maintain
the flow of patients on time for their appointment, you may need to wait in the waiting room for
an extended period or be rescheduled for another date/time.
Consultations:

Consultation fees are non-refundable and will be collected when the appointment
is scheduled. -
The consultation can be rescheduled once and still have the fee apply toward the
procedure of interest. -
However, cancellation or rescheduling within 2 business days of your appointment
will result in loss of that fee. -
The consultation fee will apply to any surgery deposit or in-office treatment deposit
placed at the time of the consultation.
Nonsurgical procedures:

A 50% deposit will be taken at the time of scheduling to secure your appointment.

Cancellations or rescheduling within two business days of your procedure(s) will
result in loss of your deposit or debit of a treatment of a prepaid package. -
All packages and deposits expire in one year and are non-transferable.

Cancellation of a package. A refund may only be given in the event of a medical
condition that prohibits treatment; written verification from a physician is required. The
refund is subject to a 3% processing fee if the original form of payment was by credit
card.
Surgical procedures:

All surgeries require a 20% non-refundable deposit. Surgical scheduling requires
careful planning and coordination between our office, the surgery center, and their
operating room staff, as well as the anesthesiologist, if applicable. Surgery deposits are
not transferable to nonsurgical treatments. -
A 10% rescheduling fee will be incurred if the surgery is rescheduled within four
weeks of the surgery date. Surgery can only be rescheduled one time. -
Payment of surgeon’s fees must be received in full by cash, check or credit card,
three weeks prior to your surgery date. -
Cancellation of your surgery within two weeks of the scheduled date is too late for us
to schedule another patient for your reserved time, therefore your payment will not be
refunded, nor will it be applied to a rescheduled procedure. -
The estimated operating room fees, facility and anesthesia, are paid by you directly
to the surgery center and anesthesia group and are given as a courtesy. The actual time
may vary based on the complexity of the case or the time it takes for you to safely
emerge from anesthesia. The surgery center or anesthesiologist routinely increase their
fees.
